Adam: Do you need a friend to talk to? Are you feeling well?
Lily: (confused as to why he is asking her this question.) Why do you say so?/Why are you saying this?/Why did you say so?

Which is the more natural expression?

None of them are natural. We would say, “Why do you ask?” or, “Why are you asking?”

The questions are all right. The important thing is that ‘it’ refers to a question.
