Is there a specific form to fill in or do you have to write a letter?
This sentence might form part of an application:
I would be grateful if you would provide me with a ‘No Objection Certificate’ so that I can apply for my passport.
Note that you don’t issue a passport - the relevant authority does the issuing. You apply for a passport. With luck, it will be issued to you.

Such questions as ‘explain where to use ’ or ‘tell me how to use ’ are generally far too vague to be able to provide useful information. Apart from anything else, there are so many ways in which the same word can be used.
