It is more of a rhetorical question, so he does not expect a response. I think he is talking to himself while he decides what he wants.

A question mark is the right punctuation.

I think β€˜Do I’ is always a question.

Same with:
Can I
Will I
Shall I
Can you
etc

Number 2 is definitely correct, but the man doesn’t say β€˜or’ with 'pants, and therefore the last β€˜or’ must be omitted. So the sentence is as following:

  • Oh, uh, do I want a sweater, or do I want some shirts, or some pants, some socks?
