obliterate/tamper

  1. The document is obliterated.
  2. The document is obliterated by someone.
  3. The document is tampered.
  4. The document is tampered with.
  5. The document is tampered with by someone.
    Please correct all sentences.

They are all grammatically possible except for #3. However, in all cases it would be much more usual to say “The document has been…”. Also, “obliterated” seems an unlikely verb to use here. “destroyed” or “defaced” is more likely.
