Is this correct?

I would said “to go to San Francisco.”

You could say “to have gone to” but that doesn’t sound nearly as natural.

For this American, “to have been to” doesn’t work.

Going to San Francisco was a good decision.
.

So when do we use ‘to have (been) + verb3’ ?
