I could add a possibility or two.

“I would appreciate it if you would help me.”

“I would appreciate it if you could help me.”
Here, “could” encompasses not just the physical ability to help but also the time and inclination to help.

Of all these, I like the “could” form best. I think it does the best job of putting forward a “polite and conditional request”.

In actual usage, this would probably be simplified.

I’d appreciate your help.

This is often a question as much as a statement. It is like saying:

Could you help me? I’d appreciate that.
