‘Really’ emphasizes ‘want’. So they want to go a lot.

Sometimes people will say ‘really’ multiple times emphasize it even more.

I really really really want to go to the movie.

It’s exactly the same as the first usage.

“I really want…”
“I really don’t want…”

Except the second one is expressed as a negative.
Consider what it would mean if you used “very much” rather than “really” in both places.

To express this emphasis, “really” is really handy.
