‘As well (as)’ means also/too. ‘As well’ goes at the end of a sentence, only. ‘As well as’ is placed at the beginning or in the middle of a sentence:
[i]Please can I bring John as well?
Can I have a cake as well as a biscuit?[/i]

You asked for the difference between these two. As well means also/in addition. This sentence might help:
They didn’t realise she wanted to come with them as well (in addition) because they left without her.
As well as is a comparative form of the adverb well. Look at this sentence:
Although the team played a good game, they didn’t play as well as they did last year In other words the game this year was not as good as the game last year.

I should have said: ‘as well’ goes at the end of a clause (not sentence, sorry), unless it means ‘with equal effect’.
Example: I might as well go.
