And i would like to add some information about Q1
I think there are two possible answer
A- failed… because the sentence is in the past
D- failing… because (after) is a preposition and after preposition we must use a gerund
Anyhow as i know both answer are possible here
‘Failing’ is NOT correct. The tense required for the sentence to make sense is the past tense.
