hi, it’s me again Showtime! I would like to ask your help again to reconstruct my letter
to explain the reason for my late filing of my clearance. I hope you will render again benevolent understanding with me. Thanks and God bless! here is my sample letter:
I, Showtime from this department,would like to apologize for the late filing of this clearance due to long sick leave for almost 2 months.
At present, my diagnosis is : Pregnancy uterine 31 2/7 weeks by Utz; Overt DM, Chronic HPN, S/P CS for CPD(1994). I was advised to go on leave last Jan. 3, 2013 for symptoms of Preterm Labor.The said symptoms resolved, however on my follow up with my Obstetrician, my Blood pressure was noted to be elevated at 160/100. Thus, i was worked up to rule out a superimposed preeclampsia. I am taking now antihypertensive medications.
These symptoms prompted my OB to possibly go on leave to have a bed rest hence the late filing of this clearance.
I hope you understand the reason behind my long sick leave. Once again, i would like to take this opportunity to apologize for any inconvenience has caused the department.
