That isn’t a complete sentence, but the meaning of that phrase would typically be similar to ‘in case we (happen to) forget’ or ‘in order to prevent any possibility that we will forget’.

There is a famous poem in English, which is often read out and quoted on occasions or ceremonies to remember soldiers who have died in war. It very much reminds people that we shouldn’t forget them. The poem is therefore a reminder to stop us forgetting.
