Have you ever heard the term 'survey ticket'?

Have you ever heard the term ‘survey ticket’? My friend said that it is a term in IT meaning something similar to ‘questionnaire’. Is he right or wrong?

And, he stressed the word ‘survey’ on the 2nd syllable, not the first one. He told me he did so to stress the word ‘ticket’ in the phrase ‘survey ticket’. I’m a bit skeptical… What do you think?

In IT, a ticket can refer to a form of communication where the sender receives a number and the message can be tracked and managed. For example, on my website I have a help ticket system. It’s just a kind of email but I can keep track of messages, etc.
