Another simpler way of saying "Degree of applicableness"

Google Forms provide online surveys but the only way to have a horizontal scale is if there is a title to the left of the row of options in my case 5 4 3 2 1.

The form will look something like this

Please rate the extent to which these following statements are applicable to yourself

I prefer the window seat to the isle seat on trains and plains. [/size]
[size=75](Highly Applicable 5 - 1 Very inapplicable) [/size]
[size=150]Degree of applicableness: 5 4 3 2 1[/size]

But degree of applicableness or “extent of applicableness” is rather stiff and longwinded. I also thought of “That’s (not) me: 5 4 3 2 1” or “Youbetcha: 5 4 3 2 1” but they are too informal.

To me, ‘degree of applicableness’ is spot on here. Which better way would you suggest to point where exactly on the scale you stand? You’re supposed to choose one number out of five to show to what extent you agree with the statement (from 5—I totally agree; to 1—I strongly disagree).

“That’s (not) me: 5 4 3 2 1” or “Youbetcha: 5 4 3 2 1” just don’t work in that case (look up what they mean).
